Services are provided as ordered by the physician, and enable patients to receive restorative measures deemed necessary, in the comfort and privacy of home.
- Registered Nurses provide nursing care by the visit. Following the physician’s orders, the RN is equipped to monitor and instruct the client regarding their care. RNs also provide specialized therapies, such as IV antibiotics, chemotherapy, and total parenteral and enteral nutrition.
- LVNs provide nursing care under the direction and supervision of the assigned RN.
- Home Health Aides provide light housekeeping, meal preparation, shopping and personal care by the visit or shift.
- Therapy, Social Work Services, & Nutritional Guidance are provided as ordered by the physician; specific courses of treatment are designed to enable the client to achieve his or her optimal level of functioning.
Teamwork
As a home health client, your Plan of Care may call for the involvement of multiple disciplines. The Home Health Agency is responsible for developing your Plan of Care (in collaboration with your doctor), and then coordinating the delivery of services.
